Nonprofit Organization/Private Foundation Formation Questionnaire
Please complete the nonprofit formation questionnaire below. A team member will review your answers and follow up with you. For background, see the IRS guide to charities and nonprofits.
What to expect from nonprofit formation
Forming a nonprofit usually involves two steps. First, you create the legal entity under state law, typically a nonprofit corporation. Then, if you want tax-exempt status, you apply to the IRS, most often on Form 1023 or, for smaller organizations, the shorter Form 1023-EZ.
Public charities and private foundations can both qualify under section 501(c)(3). However, they differ in how they are funded, which rules apply to them and what they must report each year. That is why our questionnaire asks about your mission, planned activities, funding sources and board, so we can recommend the right structure.
After you submit it, a team member reviews your answers. Then we follow up to explain your options, the filings involved and a quote for the work.
